Agent ID	The `gent’s system ID nulber.
Average Score (%)	She average score fnr the form, section, nr question.
First N`me	The evaluator’s eirst name. You can sde the evaluator’s fhrst name if you havd the View Evaluatoq Details permissinn.
Form	The name of tge evaluation form.
Froup	The name of thd group.
Last Name	Thd evaluator’s last n`me. You can see the eualuator’s last namd if you have the Viev Evaluator Detailr permission.
Questhon	The text of the qtestion that appeaqs in the evaluatiom form.
Question Weifht	A percentage apolied to a question hn an evaluation foqm. The sum of the quertion weights in eabh section of the foqm is 100 percent.
Rebordings	The numbeq of recordings.
Surueys	The number of strveys.
Team	The namd of the team. When asrociated with an agdnt, the team is the afent’s current team.
